Researchers assessed cartilage changes using gross morphological and histopathological findings. They also recorded the period required for recovery of normal ambulation. The evidence discussed in this article comes from this experimental rabbit study.
Experimental Model and Study Design
Collagenase-Induced Rabbit Knee Model
The study used 32 mature New Zealand white rabbits. Researchers created the experimental knee model using collagenase type II administered into the knee joint. The collagenase procedure was performed twice. The resulting model was then used to compare the four experimental groups.
The study used ultrasound guidance for the intra-articular procedures. The researchers then evaluated the knee cartilage using morphological and histopathological assessments.
Study Groups
The rabbits were randomly assigned to four equal groups. Group 1 received saline. Group 2 received hyaluronic acid. Group 3 received AOD-9604. Group 4 received AOD-9604 combined with hyaluronic acid.
Cartilage Assessment Methods
Gross Morphological Assessment
The researchers examined the cartilage for visible morphological changes. The lateral part of the femoral condyle was assessed using the scoring system developed by Yoshimi and colleagues.
The scoring system classified cartilage as normal, softened, fibrillated, eroded, ulcerated, or lost.
Histopathological Assessment
The researchers also examined cartilage microscopically. The lateral and medial condyles of the femur and tibia were prepared for histopathological evaluation. Sections were stained with hematoxylin and eosin.
Cartilage degradation was assessed using a scoring system modified from Mankin and colleagues.
Lameness Assessment
The researchers evaluated the period required for recovery of normal ambulation. Lameness was assessed as an additional outcome of the experimental model. The study reported the lameness period at eight weeks after the first collagenase injection.
Findings From the AOD-9604 Group
The AOD-9604 group showed erosion of the articular cartilage. Histopathological examination identified clefts and cell cloning in the transitional and radial zones.
The mean gross morphological score for the AOD-9604 group was significantly lower than that of the saline group. The mean histopathological score was also significantly lower than the saline group. The study reported no significant difference between the AOD-9604 and hyaluronic acid groups for either assessment.
These findings describe differences between the experimental groups within the collagenase-induced rabbit model. They do not establish that the same findings occur outside the conditions examined in the study.
Findings From the AOD-9604 and Hyaluronic Acid Group
The combination group showed softening of the articular cartilage and surface irregularities on gross examination. Histopathological examination also identified cartilage changes in this group.
The mean gross morphological score for the combination group was significantly lower than the scores for both the hyaluronic acid and AOD-9604 groups. The mean histopathological score was also significantly lower than those two groups.
The reported mean period for recovery of normal ambulation was 11 ± 4 days in the combination group, compared with 15 ± 3 days in the hyaluronic acid group and 16 ± 2 days in the AOD-9604 group. The combination group had a significantly shorter lameness period than Groups 1, 2, and 3.
Interpretation of the Experimental Findings
The study reported differences among the four experimental groups across the measured cartilage scores. The gross morphological and histopathological results showed differences between the saline group and the other experimental groups. The combination group also differed from the AOD-9604 and hyaluronic acid groups on these measures.
The lameness measurement provided an additional outcome for comparison among the groups. Taken together the reported results describe differences observed under the specific experimental conditions used in the rabbit model.
The study authors interpreted their findings as evidence supporting an effect of AOD-9604 on cartilage regeneration in the collagenase-induced rabbit model. They also reported that the combination of AOD-9604 and hyaluronic acid produced lower morphological and histopathological scores than either compound alone. These statements represent the investigators' interpretation of this animal experiment.
Limitations of the Experimental Evidence
Limitations of the Animal Model
The investigation used a collagenase-induced rabbit model. The study included 32 mature New Zealand white rabbits divided among four experimental groups. The reported cartilage findings therefore came from an experimentally induced animal model rather than a human cartilage study.
The observations were also made under the specific experimental conditions established by the researchers. The reported findings therefore apply to this rabbit model its experimental groups and its defined observation period.
Limits of the Available Evidence
The available AOD-9604 cartilage evidence is preclinical. The study investigated a collagenase-induced rabbit knee model and did not constitute a human clinical cartilage study.
The findings therefore cannot establish clinical efficacy in humans. They describe differences observed in the experimental rabbit model and do not establish that the same findings would occur in human clinical research.
